| Vaccine Requirements |
| To ensure all of our guest's safety, all pets entering the facility for any service must have proof of vaccines from their veterinarian's office. Canines must have current Rabies, Distemper, Parvo, and Bordetella vaccines. Bordetella must have been given within six months of reserved boarding. Felines must have current Rabies and Distemper vaccines. We highly recommend that all pets be on some form of flea and tick preventative. Any pet that is found to have external parasites upon the pre-entry evaluation will be treated at the owner's expense. Records faxed in advance to 941-921-4395 are greatly appreciated. ACE Pet Resort reserves the right to refuse admittance to pets lacking proof of vaccines, pets displaying potentially contagious conditions, and/or aggressive behavior. |

 Pre-entry Evaluation Warranty Program-------------$3.50 per pet, per visit
Upon arrival, all canine and feline guests shall be given a mandatory pre-entry evaluation in front of the owner. The evaluation checks for any problems that may affect the pet during his or her stay. The pre-entry evaluation performed by an ACE team member is superficial only and not intended to replace a physical exam performed by your veterinarian or imply medical judgments. ACE Pet Resort then "warranties" the pet's health during their stay and for three days after the pet leaves the resort - certain restrictions apply - ask an ACE team member for all of the details.
Special Needs
Specialty meals (anything other than pet food) -$1.50 per day. Any food that is not either
pre bagged in Ziplocs or in a sealed container will incur storage charges -$1.00 per day.
Medication
We will administer medication provided by the owner. All medication must be in its original container with the prescribing physician’s label indicating the name of the animal, name of the medication, the dosage and frequency. Pets requiring more than two medications per day or diabetic injections -$3.00 per day |

| Bedding and Toys |
| We provide clean and comfortable bedding for all of our guests. You may bring your pets own bedding as long as it is machine washable. Toys are also welcomed. We will do our best to return all toys and bedding upon your pet's departure. Please note that some animals destroy their bedding and/or toys when boarding. |

 Holidays
We accept holiday and/or peak season reservations with a credit card guarantee. We request a 72 hour notice of cancellation - any reservation cancelled within the 72 hour period is subject to additional fees.
Medical Emergencies
In the event of a medical emergency, we will make every effort to contact the pet's regular veterinarian first, if unavailable, we will contact one of our veterinarians.
Rates and Boarding Charges
All rates are per day and begin on the day of arrival. You will not be charged for the last day's stay if you pick up your pet before 11:00 a.m. Any pets picked up after 11:00 a.m. will be charged for that day. All rates are subject to change without notice. |

| Contract |
| Upon signing the Boarding Agreement and Warranty Program Agreement, the owner agrees to abide by and accept ACE Pet Resort's policies and procedures. Thank you for entrusting your pet with ACE Pet Resort. |
HOLIDAY RATES
An additional $3 increase per day will apply to all holiday boarding fees, beginning the day prior to the holiday through to the following day after the holiday.
